In 934, with the aid of illithid skilled in the art of morphing, the esteemed yuan-ti high priests of Tiamat subjected one of their own to a perilous transformation process. Ursorehor, a gifted yuan-ti abomination and a cunning politician of Aebilchunus, was among the few to survive this harrowing ordeal, emerging as a yuan-ti anathema. As Ursorehor was advancing in years, she accepted the offer of prolonged life spanning thousands of years, becoming Tiamat's agent of change in the process.

In 1401, distressed by the progress of Gurutharni's scaly ones, Tiamat dispatched her agent of change into the jungles of Ma'Ohari. Ursorehor was entrusted with a broad mission: unite Tha'lith's quarrelsome Yuan-ti tribes, suppress the cults of Neld-Rac, and disseminate her word among the populace. Tiamat gambled on the sheer might and presence of a figure from Yuan-ti lore; the arrival of a yuan-ti anathema signified divine favor, sending them a champion to vanquish their foes. It took eighty years to unite the tribes, a duration Tiamat had promised would be achieved only through unwavering commitment to all aspects of the mission. However, a loyalty issue arose as Ursorehor gained power and more subjects, gradually transitioning away from serving as Tiamat's mortal lackey.

During the bloody conflicts to unite Gurutharni's peoples, Ursorehor was approached by agents of Lukoon and Neld-Rac. They requested nothing in return, an arrangement that appealed to Ursorehor. She desired the freedom to choose the power she preferred, rather than serving as a pawn in a larger scheme. As is customary in such matters, Ursorehor played a role in a more expansive plan, preventing Tiamat from securing a foothold in the region.

On 23 Temporal 1478, Ursorehor established the Malshirk'iss Empire, assuming the illustrious title of Serpent Lord.
