In the year 116 GE, in service to the demon lord Athroond, aquatic demons and aboleth under the command of Blibdoolpoolp constructed Oomkaan. Situated in a region of lakes, the drainage basin of the surrounding area, they built a partially submerged citadel, naming it in tribute to the chief surgeon who served Blibdoolpoolp.

Oomkaan originated as a processing facility for captives and slaves bound for the Toadoolp Twists. As the ancestral homeland of the Kuo-toa race, Oomkaan is an archaic city where Blibdoolpoolp elected to part ways with her former master and subsequently ascend to the position of empress and deity.

After their numbers swelled to approximately 10,000, acting upon the directive of their progenitor Blibdoolpoolp, Oomkaan's forces embarked on a brutal purge across Blibdoolpoolp Bosom, eradicating and dispersing all the aboleth inhabitants.

For the initial portion of the Lith-Crillion Era, Oomkaan's prominence over other strongholds of Blibdoolpoolp Bosom derived from a region known as the Ovaries of Oomkaan. These vast pools, numbering fifteen in total, serve as taps into the dark seas of the Cresting Spires. Fish factories have operated within these otherworldly waters since the era of Blibdoolpoolp, ensuring a well-nourished populace.

In the year 262 of the God Era, Oomkaan was established as the capital city of the Âkhi Empire.
