Belgunth Bloodtusk, the daughter of Jairall Bloodtusk and the succubus Vivalin, rose to power in the foreboding swamp Nelthack. Serving under the yuan-ti dictator Miivid's army, she rapidly ascended the ranks. In 1741, Belgunth attempted a military coup, but her efforts were unsuccessful, leading to her imprisonment for life. After three decades of captivity, she teetered on the brink of demise. Belgunth then implored Rioch Tetrax, who presented her with a choice: eternal servitude or undeath to combat the malevolent Miivid. She opted for the latter.
On a dark, moonless night on 17 Lunar 1771, the moon Deaths Kindle shed its purple aura over the lands of Bal-Kriav. That night, the prison housing Belgunth was attacked by spectres, wraiths, and ghosts. A wraith killed Belgunth, adding the victim to what became known as the Roving Death Squad. This wraith was no ordinary undead, but an unholy creature infused with powerful negative energy, carrying the blessing of Rioch Tetrax. Belgunth was imbued with this blessing and rose the following night as a Taln'nazân Wraith-Queen. She proved her sinister cunning and combat prowess, ascending to lead those who had slain her. Exploring the tombs and graveyards of the yuan-ti, she set about raising a horde of skeletons, zombies, and ghouls. Her necromantic powers grew, and her fetish for spreading disease earned her a fearsome reputation among the ruling Miivid family. This time, it was the sibling of her previous employer who would face her. Many battles were fought between the Tomb Bearers (Belgunth's army) and the armies of Miivid the Terrible. Those who fell joined the ranks as undead soldiers of Belgunth. In 1790, Belgunth captured Sakullêth.
