In 862 HE, the Helas Vessels entered Bal-Kriav's upper atmosphere. These ancient ships carried the remnants of the Horgon Empire, the survivors of Suellk's ravenous mind flayers. Aboard one of these Helas Vessels was the then mortal Rioch Tetrax, one of three Saints of Maen who ruled over the Horgon Empire. Rioch Tetrax's voidship, now known as the Spire of Rioch Tetrax, landed in the Chen'gom sector of the Brucrumus mainland.

Rioch Tetrax and his minions established a new civilization in an area called Tûnin-Gil. The inhospitable terrain around these alien arrivals resulted in a relatively slow population growth. Dromite slaves and a handful of Durkoth masters remained on the surface to guard the ongoing underground work. However, Rioch Tetrax's people fared much better in the Underdark. Their advanced technology, superior organization, and honed military might from centuries of warfare on their homeworld quickly overwhelmed most resistance. Gal'palos's kuo-toans, Mîmêk, and other peoples were soon subjugated to serve under Durkoth masters. In less than a century, Rioch Tetrax had amassed a vast following of subjects who revered him as a deity. He ruled over these minions from a Lith-Crillion structure known as the Pyramid of Alchemy, with the majority of his subjects residing in the Underdark city of Nagdúrzol.

On 9 Kindle 1301 HE, the Primordial Splinter sent Rioch Tetrax's soul energy through the Temple of Ascension, granting him the status of a Cult Power. Among the members of the Primordial Splinter was Mephirouth, who at Crulock imparted extensive knowledge about disease and pestilence to Rioch Tetrax. Upon attaining the rank of a Higher Power, Rioch Tetrax chose Pestilence as his inaugural Divine Domain.

Rioch Tetrax's influence diminished following the Durkoth Descent (995 HE - 1355 HE). In the 1300s, the Borurthane initiated a resurgence of Rioch Tetrax worship by disseminating his religion into Ningizzida. Since then, his impact has expanded significantly due to the cultists and clergy of Ningizzida.

One of Rioch Tetrax's more unpleasant abilities is his decaying touch. This causes objects to rapidly age by a decade to a century, which for a human could be instantly fatal.
