The Krephus Passage was created during the Dawn Era. This remarkable structure came into being when a colossal primordial worm named Krephus escaped the Step of Foundation. After burrowing its way through the Web of Magic, Krephus found itself on another world. On the continent of Hezmort on Bal-Kriav, the worm tunneled downward, leaving behind the vast bore hole known as the Krephus Sink. In its wake, Krephus created a wide cylindrical passage that traverses numerous continents, descending into the Underdark beneath the oceans, and concluding in the Underdark region of Badmaer, where the worm met its demise.

At the northern terminus of the Badmaer region, where the passageway converges with the cavern complex Vogelak, lies the petrified remains of Krephus. The final 900 feet of this approximately 60-foot-wide tunnel are occupied by the fossilized form of the primordial purple worm. Upon Krephus's demise, a spell cast by Hon'palos triggered the magical petrification of her body, transforming it into stone as unyielding as diamond. Many foreign travelers traversing this northern section of the Krephus Passage remain unaware that they are passing through the petrified corpse of a legendary creature.

In the God Era, gnomes fleeing the demonic legions of Jurusalax, journeyed into Krephus Sink. They traversed the Krephus Passage to the Earth Seam, escaping via the Baruk-Agan portal.
