Bithiemeth was founded by the Ba'lith Empire. For nearly six centuries, this sacred city served as the epicenter of Coeus worship. In 1695, a usurper named Rax assassinated Coeus. The people of Ba'lith, as well as others across MidrĂȘth, were awestruck by this feat - for surely only a deity could slay a god. As a result, Raxcvillibus demanded to be crowned Ba'lith's emperor. At the time, the theocratic government was in disarray and greatly weakened from the loss of their god. The populace had lost confidence in the now powerless church. The military was already devoted to him, not merely for the slaying of a deity, but from the many decades that he had served as one of their most distinguished generals.

The Emperor Rax ascended to the throne of Ba'lith on 19 Warper 1695. His initial decree prohibited the Church of Coeus and disassembled the theocratic governmental structure that had governed the empire for the preceding six centuries.

In a Coeus Civil War (1694 - 1697), Bithiemeth served as the backbone of resistance against Raxcvillibus's rule. The city fell in 1696 after a grueling three-month siege. The defenders were ravaged by disease and dysentery, spread by Borurthane agents, which took a heavy toll on the population without the priestly powers of the deceased Coeus. After the city was stormed, no compassion was shown to Coeus's zealots.

After taking Bithiemeth, investigators were dispatched to examine the ruins for their unusually high abyssal emanations. Through their excavations, they discovered that the city's foundations were built atop the remains of thousands of demons who had met their demise at the hands of angelic blades and magic centuries earlier, during the Demon Spawn War (89 GE - 269 GE). This epic conflict pitted the forces of Graz'zt against those loyal to Kossuth, culminating in a decisive battle at the site of Bithiemeth. Graz'zt's army of 3,000 demons was utterly decimated, their remains reduced to ashes and bone through Kossuth's mastery of elemental fire.

By the year 945, the pile of bones and ashes had been interred beneath mounds of soil, brush, and vegetation. Only after extensive excavation did they recognize they were constructing upon a burial ground. Regrettably, by that point it was too late, as the workers, enthralled by mystical whispers, persisted in establishing a new settlement.

As a consequence of the region's persistent corruption, Ba'lith relinquished this ruin to the wilderness. Several years after the site was forsaken, the macabre and horrific demises of Bithiemeth's guardians sparked the spontaneous generation of a colossal spectral remnant. This formidable undead entity is perhaps Bithiemeth's most perilous custodian.
