In 1900 LE, impoverished Yuan-ti and their troublemaking Lorhazi compatriots were driven from the great raft city of Hazarâph in Ikhen. These exiles journeyed westward, following Galathien's southern coastlines and then trekking overland to Sinarag. In this tainted swampland, they faced threats from those unwilling to share the land. Equipped with substantial firepower and trained soldiers, the exiles held their foes at bay as they constructed a fortress on more solid ground. Their Lorhazi leaders then devised plans to bring the surrounding populace under their dominion. The majority of the construction was carried out by the Yuan-ti, who built in the style of the eastern ziggurat strongholds of Baziranâth and Loothu'n. The settlement was named Nagapolis, appeasing the vanity of their scaly rulers, the self-styled Daughters of the Marilith.

Nagapolis rests along the banks of the ebony Nithiach River. Constructed upon and around six hills, three of which are artificial. The earth and stone utilized in its construction are extracted from beneath the surface and transported via barge. Situated within the Troll Bogs, a region constantly shifting and appearing to sink, the city requires an ongoing influx of materials to fortify areas at risk of submerging beneath the swamp's murky waters. The city walls stand 120 feet tall, with an additional 80 feet of sunken foundation; city engineers are perpetually engaged in repairing the sinking infrastructure.

Nagapolis is a haven for aquatic denizens, constructed with their comfort as the primary consideration. The city boasts an abundance of canals, swampy parks, and pools. For those unaccustomed to the watery environment, traversing the swampy areas or swimming in the pools and canals can prove hazardous. Consequently, the city features several elevated levels where outsiders unfamiliar with navigating the water can reside or conduct business.

The city's population consists primarily of trolls, Graagvrii, Lorhazi, and Yuan-ti. The naga of the city, the Lorhazi, are regarded as the most privileged, with the Yuan-ti following closely behind.

The Ningizzida Empire designated Nagapolis as its capital in the year 1072.
