In the Great Exodus (998 - 1017), Tragarans from Gulimbor began migrating to the southern coasts of the Lands of Purity. By 1067, the growing power of the Farinteen Dynasty posed a serious threat to the peoples of Sinarag. Fighting the enemy as disparate tribes or disorganized groups would ultimately lead to their downfall unless they united. A cunning naga named Ning'izzi worked to break the tribes and bring them under her control. The leaders of the numerous evil groups dwelling in the area's wetlands were persuaded to cease their bickering and infighting. After five years, Ning'izzi convinced them to form a confederacy. The Ningizzida empire was established in 1072, with Ning'izzi wielding near-dictatorial power. Since then, the fortunes of the empire have waxed and waned, as the challenges of maintaining the confederacy arise from the death of group leaders, political machinations, or the inherent evil of the empire's populace. In the Black Tide War (1465 - 1486), they united as one and successfully repelled the undead hordes from making inroads into Sinarag. The leader who rallied the groups once again was a resurrected Ning'izzi, raised from the dead as a slaughter wight by a group of shamans who had sacrificed their souls to Rioch-Tetrax. She remained undead for six years, uniting her people as a single, formidable force and leading them against the Black Tide, driving them back. Afterward, she was called back to the domain of Rioch-Tetrax.

The empire of Ningizzida stretches across the wetlands of Sinarag and the Troll Bogs. The chaotic denizens of this realm are loosely organized under the tyrannical rule of the self-proclaimed Empress Isheba. Isheba's evil empire is a confederacy comprising the trolls, Viidostor, Graagvrii, yuan-ti, and the Lorhazi. These constituent empires, situated in the Troll Bogs, harbor a multitude of nefarious creatures and monsters.

The hordes of Ningizzida are adversaries of Malacost and the Divine Empire. They are satisfied with pillaging, slaying, and enslaving, rather than geographical expansion.

Isheba, the leader of the Ningizzida, and her underlings pose a dire threat to the virtuous realms of the Lands of Purity. The treacherous swamps thwart any substantial military campaign against the holdings of this malevolent empire. Malacost and the Divine Empire have constructed a network of fortified outposts and uphold rigorous border patrols along the periphery of this gloomy quagmire. These measures maintain the transgressions of the serpent worshipers in a relatively controllable condition.

Two formidable disease-disseminating cults are safeguarded, if not subsidized by the Ningizzida. These cults are Sanagar-Umlâs and Borurthane.
